1. How do I get an order form for a plate?
Click on the "leaf" at the left side bar for the plate that you want. Once you are at the page for the plate you want, scroll down to the bottom of the page and click on "ORDER". The order form will be down loaded and displayed on your computer. Print your form out and handwrite information in.
NOTE: If the order form does not appear then you may not have Adobe Acrobat. However, it can be easily downloaded for free off of the internet. If you do not want to down load the order form you can e-mail me and I will send you an order form by fax or postal service.

How long does it take to get a plate?The process from when I first get your order form and payment, to the day your plate is shipped, is 2-3 weeks. This allows 2 weeks for the design and completion of the plate and 1 week for shipping.
If you need your plate earlier let me know since I may be able to meet your deadline. Customers who order the Large Ancestry Tree Plate or the Large Family Tree Plate can receive an e-mail proof of their plate to confirm all information. The 2-3 week time frame for these two plates is based on the customer returning an e-mail with their approval/changes the following day their e-mail is received. Delays in returning the e-mail can affect the shipment date.

How do I order the Large Family Tree or Large Ancestry Tree plate and what is the process in being ordered?- Complete the order form and return it with a deposit of 50% of the total cost and half of shipping if shipped. Payment can be made by cashiers check, money order or personal check. Out of state customers who need their plate in a hurry and do not want a proof should make their payment in full (with shipping) by cashiers check or money order.

- Your plate is designed.
- I will e-mail you a proof. A proof is a photo of your plate without color or detail. Upon receiving your proof, you need to verify that all names are spelled correctly, dates are accurate and that branches are put in the proper birth order on your plate. The following day reply by e-mail with corrections for your plate or give your approval.
- Out of State & Non Local Customers: Once you approve the proof by e-mail, or make changes, send the payment balance by cashiers check or money order. Your plate is shipped upon receiving your final payment. If a personal check is used, it will need to be cleared before shipping your plate.
- Local Customers: Once the plate is complete, I will phone you to make arrangements where it can be picked up. The balance is paid when you pick your plate up.
- Down load the order form from your computer.
- Complete the order form and return it with your full payment including shipping. Personal checks will need to be cleared before shipment. Out of state customers who are in a hurry should consider payment by cashiers check or money order.

- Local Customers: Once your plate is completed, I will phone you to make arrangements for where you can pick up your plate up.
- Out of State Customers: Once the plate is complete, and your check has cleared, your plate is shipped.
5. What is the difference between a Ancestry Tree Plate and a Family Tree Plate?
The Family Tree Plate is of a current family going from parents, their children, grandchildren and their great-grandchildren. Each person is put in the correct birth order and marriages include the spouses name. Remarriages with or without additional children can be included if desired.
The Ancestry Tree Plate goes back in time (up to 14 names). The plate begins with a husband and wife (2 names), next their parents on both sides (4 names), and then their grandparents on both sides of the husband and wife's family ( 8 names). First and last names are used (women use their maiden last name), and birth dates are written under each name (example: 6-28-1943). A fourth line; either the place of birth or the death date can be added.
6. Can additional names or branches be added later?
No, the tree branches and leaf detail are designed and filled in to look its very best based using the names and dates supplied when the first ordered.
